No idea how this could happen other than multiple layers of incompetence. Prop guns should always be treated as if they are loaded with live rounds.
From what I've heard, while the prop guns are not loaded with live rounds, some do contain a wadding material that holds the gunpowder in and it can be expelled from the gun with considerable force.

Well, he may be a victim. If some dumb ass props guy put a live round in a gun and handed it to Baldwin to do a scene then yes, Baldwin would be completely exonerated and I'm sure he would feel like shit for accidentally killing the woman. I don't like his politics either but if it played out this way what a thing to have on your conscious.

If anyone is charged with negligeance it's the dude in charge of props. Do we know if he was acting like a doosh shooting at people or if he was actually shooting a scene? Waiting for the details to come out, but he shot the cinematographer and the director... if he was shooting a scene, he's got really bad aim. Really does sound like he was being a doosh. Could be on him for acting like a douche (gun safety: always act like a gun is loaded, etc) and the prop guy who loaded the blanks. This. Enough blame to go around. And blanks can still kill people if you're not careful.

But wadding is not going to go through one person and into another. It was with all probability a FMJ. The bigger problem here is why they would even have live rounds in the same vicinity. Hell, even when I practice dry firing I always put my loaded magazines and any ammo into a different room.
